Anders has a plus one, the spirit Justice. Justice isn't just his name, he literally is Justice, and after Anders befriended the spirit and discovered he needed a body, Anders offered his. It's not proven to be the best decision ever. When Anders' eyes turn blue and his skin seems to crack, also showing blue, Justice has taken control. Generally it won't be a threat to patients, unless someone calls Justice a demon. Then Justice may lash out at them.
This tends to happen only if Anders is very angry or in direct danger or both. Justice takes over and then sees everything in black and white, with no shades of grey. He cannot abide anything that he sees as an injustice and will act against it. The best way to deal with Justice is to have someone who Anders respects talking to him, getting him back under control, but should there be no one like that, backing away/down is the safest option, and Anders will regain control in time.
